Surgery for Perforated Peptic Ulcers at Prachomklao Hospital

AbstractThe results of surgical treatment of perforated peptic ulcers in 330 patients, during the 12 year period from October 1989 to September 2001, were analysed. One hundred and ninety two patients were treated by simple closure and 138 patients by definitive surgery. There were no diffe rences in age incidence, occupation, state of shock upon admission and duration of symptoms be tween the two groups. Complication rates were 30.21 and 19.57 per cent in simple closure and the definitive surgery group respectively. Mortality rate was 1.56 per cent in simple closure and no death in the definitive surgery group. Thirty-four patients in the simple closure group required definitive surgery subsequently for repeated perforation. It may be concluded that, except for perforation of acute duodenal ulcer, definitive surgery should be the treatment of choice in patients with a perforated peptic ulcer. Parameters used to decide whether to perform definitive surgery include the patient's condition and experience of the surgeon and surgical team.
